Actually to save the time from people having to email me to get a copy of my forms and how I sent them in, here are the instructions I followed:

1. claim amount: UNLIQUIDATED
- do not check box

2. basis for claim: BREACH OF WARRANTY AND MAINTENANCE AGREEMENTS

3, 3a, 3b. leave blank

4. leave blank

5. leave blank

6. leave blank

7. "If the documents are not available, please explain:" DOCUMENTS WILL BE PROVIDED UPON REQUEST

8. check box "I am the creditor"

Fellow owners, submit your claim forms! It is more important that you submit them than that you fill out every detail correctly (although Hockeydad has provided the correct answers from a bankruptcy attorney's point of view, making it easy). Don't be left out of the settlement, if there is one for the owners.

Yes, don't forget it's 2 Proof of Claim forms, one for 13087 and the other for 13086. I have emailed people a copy of my submission but they need to remember to fill out both forms.

Amount: I really enter unliquidated? or the value of my purchased car?
What does it mean UNLIQUIDATED?

That just means that the amount is not fixed yet because we don't know how much the warranty repairs on our cars will ultimately cost.

Well, I guess $2k IS better than nothing. I see all this info re: filling out the form, but can't find the actual form. Can you post a link to the site?

I have a question, if I fill out forms since I have had out of pocket repairs, if Fisker does re-open and honor our warranties will this disqualify our cars for future warranty work since we took the settlements?
Also where do I need to overnight the forms too, address that is?
I too need a copy of the forms, where are the located, or do we have to email someone?
Thanks for any info in advance, I will try to get them out tomorrow....Biggs

Shoot - my thing didn't have a suite # - hope that's not a problem!
